ارتباط پزشک و بیمار در شبکۀ روابط میان‌فرهنگی تجربۀ درمان در محیط‌های چند‌فرهنگی بیمارستان‌های دولتی تهران

چکیده

حوزۀ ارتباطات سلامت، یکی از حوزه­‌های در حال رشد است که مفاهیم سلامت را به ارتباطات پیوند می‌‏دهد. یکی از بخش­‌های مهم این حوزه، رابطۀ پزشک و بیمار است. موضوع اصلی این مقاله، بررسی رابطۀ پزشک و بیمار در بستر ارتباطات فرهنگی است. با توجه ‌به ­الگوهای ارتباطی مختلف که در رابطۀ پزشک و بیمار حاکم‌اند، پیش‌بینی می‌شود الگوی ارتباطی حاکم در نظام درمانی، الگوی پدرسالارانه یا والدینی باشد. در این تحقیق که به روش مصاحبۀ کیفی انجام شده و داده‌­ها با استفاده از نرم‌افزار مکس کیودا تحلیل شده­اند، برای رسیدن به هدف تحقیق، با 15 بیمار که دارای قومیت‌ها و گویش‌های متفاوت بودند و همچنین 10 پزشک و اینترن و رزیدنت که در سال 1398 تجربۀ کار را در بیمارستان­‌های دولتی شهر تهران داشتند، مصاحبه‌ انجام شد و نتایج آن در سه مرحله کدگذاری و تحلیل شدند. در هر دو گروه، 3 دسته کد فراگیر شامل اختلالات ارتباطی اطلاعاتی، اقتدار پزشک و اختلال در ارتباطات میان­ فرهنگی به­ دست آمد. با توجه ‌به تحلیل و بررسی یافته­ های تحقیق، این نتیجه حاصل شد که اختلال ارتباطی در رابطۀ پزشک و بیمار به‌طورعام وجود دارد و مسئلۀ تفاوت فرهنگی، تنها یکی از عوامل تشدیدکنندۀ اختلال ارتباطی میان پزشک و بیمار و در حاشیۀ این ارتباط است. همچنین این نتیجه حاصل شد که نظام درمانی درحال‌ حاضر درزمینۀ مهارت­‌های ارتباطی، ضعف‌های زیادی دارد و الگوی ارتباطی پدرسالارانه بر آن حاکم است و خلأ آموزش دانشگاهیِ مهارت­‌های ارتباطی به جامعۀ پزشکی و همچنین جامعۀ بیماران، مهم‏ترین عامل این ضعف است. با توجه ‌به تحلیل مصاحبه­‌های پزشکان، می‌توانیم پیش‌بینی کنیم با ورود نسل جدید و جوان به جامعۀ پزشکی، الگوی ارتباطی پدرسالارانه، به الگوهای مشارکتی و هدایتی تغییر شکل دهند و شاهد خلأهای ارتباطی کمتری در محیط درمان باشیم.

چکیده [English]

The main topic of this article is to examine the relationship between physician and patient in the context of cultural communication. According to the different communication models that govern the relationship between physician and patient, it is predicted that the dominant communication model in the treatment system will be patriarchal or parental model. The method of this research is qualitative and the data are analyzed using Maxqda software. To achieve the purpose of the study, 15 patients with different ethnicities and dialects, as well as 10 physicians, interns and residents who had experience working in public hospitals in Tehran in 1398 were interviewed. Then the interview results were coded and analyzed in three stages. In both groups, 3 categories of comprehensive code including communication-information disorders, physician authority, and inter-cultural communication disorders were obtained,. According to the analysis of the research findings, it was concluded that communication disorder in the relationship between physician and patient in general and the issue of cultural difference is only one of the aggravating factors of communication disorder between physician and patient and is on the margins of this relationship. It was also concluded that the treatment system currently has many weaknesses in the field of communication skills and is dominated by the patriarchal communication model. The lack of academic communication skills in the medical community as well as the patient community is the most important factor in this weakness.
